Abstract

Purpose

Uganda is one of many African countries struggling to develop adequate healthcare, particularly in regard to local treatment and access to drugs. The purpose of this paper is to contribute to the understanding of how reducing supply‐chain complexity can improve health in developing countries.

Design/methodology/approach

This study and evaluation included 50 interviews and 27 site visits of the public healthcare system in Karamoja, northeast Uganda. A mapping of drug‐supply chains was undertaken to identify causes of stock shortages and possible solutions. A model for logistics process redesign was used for the analysis. Results were quantified with use of a simple tool developed for this specific purpose.

Findings

The main conclusion is that less supply‐chain complexity can produce higher customer service in terms of less stock shortages, while keeping costs down. By reducing lead times and uncertainty, increasing order frequencies, and moving order points and safety stocks, there may be better integration between information and goods flows and bottlenecks in the supply chain may be reduced.

Research limitations/implications

While the empirical study is extensive, there are uncertainties in the data that must be taken into account. The effects of the suggested solutions remain to be analyzed and documented upon implementation.

Practical implications

The study was rooted in a practical problem and provides practical solutions for developing countries and agencies providing aid.

Social implications

Stock shortages of life‐saving drugs are a general problem in countries with lack of financial and technical infrastructure. Improvements will impact the lives of many people.

Originality/value

The paper provides an understanding of the applicability of traditional logistics principles in a new context. It provides the academic community with a much‐needed in‐depth understanding of humanitarian logistics. The approach can be used in other studies.

Abstract

Purpose

The purpose of this paper is to report on a qualitative study of the learning and development of 70 external mentors during the first year of their deployment to support early career teachers’ professional learning as part of a national initiative aimed at school improvement in Wales.

Design/methodology/approach

The study adopted a narrative methodology that elicited accounts of external mentors’ learning experiences that were captured as textual data and analysed using an inductive approach to identify: first, the manifest themes that appeared at declarative level, and second, the latent (sub-textual) themes of external mentor learning and development.

Findings

Four key themes emerged that indicate the complexity of transition to the role of external mentor in high-stakes contexts. From these, eight theoretically-informed principles were derived which support mentors to embrace uncertainty as essential to their learning and development, and to harness the potential they bring as boundary-crossers to support the development of new teachers.

Research limitations/implications

The study investigated the first year of a three-year programme and worked with one form of qualitative data collection. The research results may lack generalisability and a longitudinal study is necessary to further explore the validity of the findings.

Practical implications

The eight principles provide a foundation for mentor development programmes that can support ambitious goals for mentoring early career teachers.

Originality/value

The study addresses the under-researched area of the learning and development of external mentors at a national scale.

Purpose

Technology enabled care (TEC) equipment has been a part of adult social care (ASC) in most areas of the UK for decades. More recently, commissioners have been exploring mainstream technologies’ potential to support older people living in the community. The purpose of this paper is to examine the challenges and opportunities presented by the growing application of mainstream technologies in ASC against the backdrop of the planned UK-wide switchover from analogue to digital telecommunications by 2025.

Design/methodology/approach

This general review uses research evidence, literature, industry standards and policy documents to address the challenges presented by the UK’s digital switchover and the potential role of mainstream technologies such as “smart speakers” in ASC.

Findings

The use of digital TEC and mainstream devices in ASC brings challenges related to access to reliable internet connections in parts of the UK; the need to update TEC safety standards; the ethics, privacy and data security measures that ensure digital products and services safeguard the interests of users; and evidence and commissioning practice.

Originality/value

This general review explores emerging areas of policy and research related to the planned UK digital switchover and the use of mainstream technologies in ASC. It offers a conceptual approach and suggests ways forward for research, regulatory and commissioning agendas.

Abstract

Purpose

Understanding patients' experiences of their interactions with health services is an important step in building quality from within. The purpose of this article is to look at the possibilities for involving service users in the development of the National Health Service in England through the structure of integrated care pathways (ICPs).

Design/methodology/approach

A systematic literature review was undertaken to identify how patient experiences have been attained and used in three clinical areas: cataract care, hip replacement and knee arthroscopy. The information was weighted according to methodological criteria and synthesized according to the typical stages of each pathway. Key issues were summarised thematically across each pathway.

Findings

The findings relate to the use of patient views and experiences within organisational structures, service development, methodological research, education and training. The article identifies important issues of practical significance for involving service users in the planning and development of patient focused ICPs: such as the diversity of patients, perspectives of continuity, information and patient support and the need for methodological research.

Research limitations/implications

The review is limited in that the literature across all three pathways tends to report findings of small studies undertaken in one clinical service or setting and most studies are not randomised or controlled.

Originality/value

The literature identified by the review contains important messages for both NHS policy and future research to involve service users in the planned expansion and plurality of NHS care.
